Your co-payment is due at the time of service. To expedite the check-in/check-out process, we will collect your co-payment at the time of check-in.

In the event you are unable to keep your appointment, we require a 24-hour notice or you may incur a $50 cancellation fee for office visits and a $100 fee for surgery/procedure appointments. Notifying us in advance will enable us to accommodate the many requests we receive daily from other patients for immediate/sooner appointments.

To better serve all our patients, we reserve the right to reschedule your appointment if you are more than 15 minutes late. Please call ahead if you may be late, and in turn we will do our best to be considerate of your valuable time. Please understand that emergencies do come up and take precedence over routine appointments and may cause a delay in attending to scheduled patients.

While we will do our best to aid you in resolving any insurance issues such as referrals, co-insurance or deductibles, it is your responsibility to know your insurance benefits, requirements and coverage level. We cannot make any concessions if any of these circumstances arise and you will be responsible for any costs incurred for services provided.
